Job Summary:
- Responsible for providing exceptional customer service while participating in the discharge planning process.
- Facilitates, and coordinates delivery of agency services by ensuring accurate demographic information and appropriate physician orders.
- Assumes responsibility and accountability for facilitating, communicating, and collaborating with both the healthcare team, and the patient/family to ensure an outstanding customer experience.
- Promotes the optimal health, well-being, and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines, and the State Nurse Practice Act.

Education:
- Graduate of an accredited Licensed Practical Nursing program.
Certifications/Licenses:
- Current state/Compact LPN License.
- BLS, CPR required.
- LPN IV certification preferred.
Experience:
- Two years of recent LPN experience
- One year of home care, hospice or home infusion experience preferred.
- Previous experience in a discharge planning, preferred.
