We serve doctors by offering Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services.
Our goal is for patients to receive personalized and frequent preventative care from the comfort of their homes and away from healthcare facilities.
Position: Clinical Specialist: CCM
The RN or LVN is responsible for assisting with the chronic care management program.
Responsibilities
- Assisting patients from referral to start of care.
- Completing an in-depth comprehensive care plan for new patients.
- Calling patients, as needed, to check in on them and provide advice within scope of practice.
- Addressing patient concerns in a timely fashion.
- Fulfilling monthly care time with each patient they are assigned to.
- Perform care coordination between our team and the referring provider’s office, other providers that are an active part of the patient’s care, and available community resources.
- Promptly identifying and communicating patient concerns with Physicians and being responsive to our Physicians in a timely manner.
- Work closely with the RPM team. Ensuring patients utilize their monitoring equipment as instructed, reviewing vital sign trends with patients, and closely coordinating care between the RPM and CCM care team.
- Performing complete and accurate data entry of qualifying services and results into database software and EMR.
- Acting as a strong care advocate for all patients on service.
- Other duties as assigned.
Non-Negotiable Requirements
- Must have an active RN or LVN certification issued by an accredited provider.
- Must be comfortable with computers and learning new software systems.
- Must be comfortable navigating and working between different software systems, EMRs, and spreadsheets when working with a patient.
- Must have in-depth knowledge and experience with medical terminology.
- Must be tech savvy with “smart” devices and have patience and willingness to problem solve technical issues as they arise. Our devices are simple, but our patients are all Medicare age and do need assistance at times.
- Must have excellent organizational skills, be able to work with minimal supervision, and demonstrate a team-oriented approach.
- Must be always professional, have a good work ethic, be a self-starter, be a conscientious problem solver, and be results-oriented.
- Must be adaptable, detail oriented, willing to learn, and take personal pride in their work.
- Must be able to work the full hours committed to each week without significant interruption.
- Must be able to work in a quiet environment free from distractions.
- Must be able to multi-task effectively.
Ideal Candidate Requirements (preferred, but not required)
- Prior working experience in a physician office setting, ideally Primary Care or Cardiology.
- Prior working experience in a home health care setting.
- Proficiency with online databases and Microsoft Office products.
- Experience working in EMRs.
Job Types: Full-time, Part-time
Pay: $28.00 - $30.00 per hour
Expected hours: 20 – 32 per week
