Responsibilities

• Perform in-home and ensure visit time fully meets the needs of the patient.
• Coordinate total patient care including interdisciplinary communication with other health care providers to enhance continuity of care.
• Render skilled nursing care, as assigned by the registered nurse, according to the physician's orders.
• Recognize and interpret symptoms; report patient's condition and assist with or institute remedial measures for adverse developments.
• Guide and instruct patient and their families in prescribed activities that are directed toward improving independence and functionality.
• Ensure lab tests are obtained, stored, and transported according to acceptable laboratory practice and OSHA guidelines.
• Review and update the medication profile; assess patient compliance, understanding of medication regime, contraindications, and side effects.

Qualifications

• Must be an LPN or LVN in the state of employment.
• Must have at least one year of experience as an LPN or LVN.
• Medicare Home Health experience is preferred.
