RESPONSIBILITIES:
Follow the plan of care, providing treatments within the scope of licensure and working collaboratively with the members of the health care team to help meet patient care outcomes
Furnish services in accordance with Attentive Home Health Services policies and procedures
Provide direct patient care as defined by the Ohio State Nurse Practice Act
Prepare clinical and progress notes and submit in a timely fashion
Provide accurate and timely documentation consistent with the plan of care
Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care
Participate in the coordination of Home Health services, appropriately reporting the identified needs for other disciplines to the Director of Nursing
Active member of the health care team and assist the physician and Registered Nurse in performing specialized procedures
Use equipment and supplies effectively and efficiently
Prepare equipment and materials for treatment observing aseptic technique as required
Participate in personal and professional growth and development
Performs other duties as assigned by the Registered Nurse or Director of Nursing
Participates in on-call duties as defined by the on-call policy
Participate in in-service programs
Assist the patient in obtaining needed equipment and other necessary items and services
Assist the patient in learning appropriate self-care techniques
QUALIFICATIONS:
Graduate of an accredited School of Practical Nursing or Vocational Nursing Program
One (1) or Two (2) years of acute care experience or recent Home Health experience required
Current license to practice nursing in the state of Ohio
CPR Certified (BLS minimum)
Valid Driver's License and required insurance
Verbal and written communication and math skills
Organizational skills to work in a fast-paced environment
Comply with accepted professional standards and practices
Able to lift, position or transfer patients
Able to lift fifty (50) pounds - reaching, stooping, bending and kneeling are required
Must have visual acuity and hearing to perform required nursing skills
Maintain required continuing education requirements needed for licensure or certification
Must drive in various weather conditions or roads in varying degrees of repair and maintenance
Able to travel and a dependable vehicle required
Able to handle emergency or crisis situations

Salary: $45,000.00 - $75,000.00 per year
