New York Cancer and Blood Specialists (NYCBS), a prominent and respected Oncology/Hematology group, is seeking a LPN (Chronic Care Management) to join a well-established and growing pure sub-specialty practice with academic affiliation. Practice manages a freestanding outpatient Cancer Center with extensive chemotherapy administration, Radiation Oncology, Pharmacy and Research department..
The Chronic Care Management (CCM) LPN is responsible for the care coordination of Medicare and Medicare Advantage patients through telehealth.
In this role you will:
- Communicate with patients monthly via telephone and update pertinent health status changes in OncoEMR
- Review and reconcile medications on a monthly basis
- Escalate symptomatic patients to Triage for further assessment
- Collaborate with careteam to help ensure positive patient outcomes
- Maintain accurate and clinically appropriate documentation
- Provide patient education with respect to medications and disease management
- Adhere to HIPAA guidelines while handling sensitive patient information
- Current NYS LPN license required
- Consistent professional conduct and meticulous attention to detail
- Excellent verbal and written communication skills as well as excellent interpersonal skills with patients, staff, and other health care professionals
- Bi-Lingual, English/Spanish a plus
- HemOnc experience preferred.
