Excel Medical Center LLC is a growing primary care practice with a mission to serve the medically-underserved adult, urban population located in the Philadelphia area via our 15 primary care clinical locations.
In addition, we provide medical management in 13 Nursing Homes/Long-Term Care (LTC) setting.
We are actively seeking a positive, caring, experienced LPN Case Manager to be based in one of our North Philadelphia primary care clinics who will have oversight and direct outreach to our patients to:
- Enrolling and onboarding patients to the Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.
- Educating patients on their device functions and checking that they can properly setup their devices. Monitor patient medication non-compliance alerts and provide the proper assistance to the patients so they maintain their medication regimen.
- Provide oversight and develop the RPM & CCM programs
- Grow with the position to eventually add oversight of Principal Care Management (PCM).
- Follow up with non-compliant patients to schedule appointments and keep them engaged in their care plan.
- Coordinate patient care with the HUMA Healthcare team, as appropriate.
- Develop strong, working relationships with the Providers to build credibility in the program and yourself to best serve the patients.
- As the program grows and we hire more RPM/CCM staff, work with your management team to see if there is an opportunity to leverage strong interactive communications and leadership skills to provide the guidance on how to best grow the program.
- Valid PA LPN License
- Minimum 1-2 years LPN primary care clinical experience
- LPN patient case management experience
- Min 1-2 years RPM & Care Plan experience
- Minimum of 1-2 years with patient outreach, preferably working with urban, adult Medically Underserved Area (MUA) patients
- Goal oriented; committed to impactful patient outcomes leveraging a positive attitude and a patient, compassionate demeanor.
- Missioned-oriented and driven to provide primary care medical services to an underserved community
- Strong interactive communication skills and leardership skills are a +
- Open to learn from all staff members while working in a very fast-paced environment.
- 15 PTO days (if position is full-time)

Focus on Wellness:
We create an atmosphere that invites our adult patients to visit us, as their health care provider, from time to time, healthy or otherwise. The purpose of these visits is to:
- Screen for diseases, such as high blood pressure and diabetes
- Search for potential disease risks, such as high cholesterol and obesity
- Counseling on such topics as quitting smoking, losing weight, eating healthy and reducing alcohol use
- Flu and pneumonia shots
- STD screening
