Responsibilities:
  • Complete medical and wellness assessments of patients
  • Provide individual and group health and wellness education including but not limited to medication, chronic disease management, stress reduction, nutrition and other general health and wellness concerns.
  • Monitor and administer medication as prescribed.
  • Work collaboratively with Primary Care practitioners and Behavioral Health team.
  • Provide appropriate health care and coordinate referrals
  • Care coordination of patient care and needs within the agency and all external providers, families, residential programs, etc.
  • Document in electronic health records per CHL policy and procedure.
  • Attend all required meetings and trainings.
  • Stay current with nursing practice.
  • Perform other job-related duties as assigned.
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.
Qualifications:
  • Grade N02: Community LPN: Current Massachusetts LPN license
  • ASN preferred
  • Experience with quality improvement process to improve client care and meet required health risk outcome measures
  • Excellent communication and documentation skills
  • 2-3 years of medical nursing experience with behavioral health and substance abuse experience preferred
  • Interest and knowledge about health and wellness, especially regarding clients with medical and behavioral health concerns
  • Work well independently and as a member of and Integrated Health Care team
  • Bilingual Preferred
  • Current valid US-issued driver's license and ability to provide registered and insured automobile for work related purposes including transporting clients
