Responsibilities:
- Complete medical and wellness assessments of patients
- Provide individual and group health and wellness education including but not limited to medication, chronic disease management, stress reduction, nutrition and other general health and wellness concerns.
- Monitor and administer medication as prescribed.
- Work collaboratively with Primary Care practitioners and Behavioral Health team.
- Provide appropriate health care and coordinate referrals
- Care coordination of patient care and needs within the agency and all external providers, families, residential programs, etc.
- Document in electronic health records per CHL policy and procedure.
- Attend all required meetings and trainings.
- Stay current with nursing practice.
- Perform other job-related duties as assigned.

Qualifications:
- Grade N02: Community LPN: Current Massachusetts LPN license
- ASN preferred
- Experience with quality improvement process to improve client care and meet required health risk outcome measures
- Excellent communication and documentation skills
- 2-3 years of medical nursing experience with behavioral health and substance abuse experience preferred
- Interest and knowledge about health and wellness, especially regarding clients with medical and behavioral health concerns
- Work well independently and as a member of and Integrated Health Care team
- Bilingual Preferred
- Current valid US-issued driver's license and ability to provide registered and insured automobile for work related purposes including transporting clients
