Locum - Physician - Maternal Fetal Medicine Boise Id

Saint Alphonsus Regional Medical Center is seeking an MFM locum to assist with 24 hour call. They are looking for coverage starting as soon as credentialed.
Schedule: 24 hour call. Must be available for one 7 day block per month. Client is flexible on availability and will work around provider's schedule.
EMR: Epic
Estimated Credentialing Timeframe: 60-90 days.
Will pay for flight, hotel, and rental car or mileage(billed at IRS standard rate)

Please make sure you have the following items readily available for your provider at the time of acceptance in order to expedite the credentialing process

• MD diploma
• Internship/residency/fellowship certs
• All medical licenses
• DEA
• Board Certification
• Life support cards- BLS
• Current and previous certificates of insurance
• Driver s license
• Professional color photo/headshot
• CMEs for the past 2 years
• Immunization records including TB, Flu, HepB, Tdap, Varicella, and COVID
• References
• Case logs

Requirement description :

• Board Certified -REQUIRED
• Idaho license or IMLC - Required
• Must meet perm rate cap of NA5k
• Must have proof of COVID-19 Vaccine - Required - Verbal Confirmation needed at time of name clear
