Lmsw- Social Worker

LeRoy Village Green RHCF is searching for a Social Worker to become a part of our facility’s compassionate care-giving team. We are a 140-bed skilled nursing facility located in the scenic Village of LeRoy located in Genesee County, New York.

Social Worker Requirements:

• Master’s Degree in Social Work
• Previous Short Term/Sub Acute Experience

Social Worker Duties include:

• Completing MDS's
• Maintain an open line of communication with residents and families
• Attending care plan meetings
• Discharge Planning
