Lifestyle Medicine Coordinator

Family Medicine & Obstetrics Practice is seeking a Lifestyle Medicine Coordinator to join a great team.

The team has a mission to serve and make a difference in each patient interaction and is seeking a Lifestyle Medicine Coordinator with a passion for service.

RESPONSIBILITIES

Lifestyle Medicine program development & coordination

• Help support the growth and development of the practice’s Lifestyle Medicine services including patient education, one-on-one support, group visits, and wellness programs.
• Utilize a Bible-based Lifestyle Medicine framework to support physical, emotional, and spiritual wellness.

Patient Education & Individual Support

• Coordinate the Chronic Care Management program & care plans for eligible members offering one-on-one support (Medicare CCM).

Group Support

• Coordinate group Lifestyle Medicine visits with Physicians / Providers facilitating the communication with patients and helping to create a positive experience.

Other clinical and administrative tasks based on experience may include:

• Registered Dietitian / Nutrition Counseling, Medical Assisting, Lifestyle Medicine Health Coaching, Community / Church Wellness Event Support.

SKILLS

• Exhibit effective communication both orally and in writing.
• Possess organizational and time management skills.
• Ability to build strong, trusting relationships with participants.
• Ability to positively resolve participant challenges and objections about their health and behaviors.
• Ability to provide empathy in difficult interpersonal situations.
• Eagerness to learn new skills, including problem solving training, exercise training and motivational interviewing.
