Pathfinders Child and Family Services, LLC is hiring Service Providers for the youth and families across the state of Ohio. Pathfinders strives to provide education and resources totoday’s youth to provide the tools necessary to lead a thriving and fulfilled life. We provide youth and families with one-of-a-kind expertise that allows them to grow in self-confidence, self-regulation and self-esteem enabling skills.
Position Summary: We are growing with OhioRise! We are looking for both Moderate and Intensive Care Service Providersto work within the counties across Ohio. We are looking for professionals that understand High-Fidelity Wraparound practice while providing services to identified youth that will provide specific, measurable, and individualized services to each person served.
Our providers develop positive relationships with the youth and families of Ohio in partnership with a system of care professionals. Our providers facilitate the development of youth and family informal supports and resources. We are proactive in system advocacy for our youth and families. The Service Provider takes lead role in scheduling & facilitating collaborative one on one sessions in the community with identified youth.
Responsibilities Include:
- Provide Life Skills to help youth to become more self-sufficient and confident in day-to-day skills. Teach coping skills and self-regulation strategies to support informed decisions, choices, and consequences.• Assist youth in development of healthy relationships and interactions with others.• Maintain required caseload of 1:12 at any given time. • Complete all required documents as outlined by Pathfinders Child and Family Services, LLC to include the Strengths, Needs and Cultural Discovery Assessment and the Wraparound plan.• Work collaboratively with identified partners on behalf of the Child and Family team to include both formal and informal supports.• Monitor the provision and quality of services provided to the family through the Child & Family Team and act as liaison when new services/resources need to be sought or developed.• Provide written and verbal information related to the youthand family based on assessment and youth contact. This information will include the youth and family’s strengths and competencies, progress, or lack of progress, as well as report on the services and supports put in place to assist the family.• Provide Wraparound services as part of the OhioRISEProject, using the High-Fidelity Wraparound model to clients and families identified. Deliver service in a variety of settings in the home and community.
Qualifications:
- Minimum High School Degree with 3+ years’ experience with youth, inclusive of those with Serious Emotional Disturbances (SED).• Ability to perform job responsibilities with a high degree of initiative and independent judgment.• Sensitivity in relating to youth of varying backgrounds and demonstrated ability to work with diverse groups of people possessing various strengths, aptitudes, and abilities.• A valid driver's license with approved driving record,personal transportation, and insurance as well as complete Fingerprint Based Background Check• 21 years and older
Job Type: Contract
Pay: $35.00 - $50.00 per hour
Expected hours: 12 – 40 per week
