- PART TWO: FUNCTIONAL RESPONSIBILITY
- Develop, implement, facilitate and evaluate the monthly calendar in conjunction with the Life Enrichment coordinators calendar.
- Develop program for memory care residents that promote wellness, maintain resident independence that are both mentally/physically challenging to residents.
- Individualize activities based on resident individualized needs as identified in resident personal history intake form.
- Communicate resident programs to residents and their families
- Plan and participate in special events
- Review the activity supplies and equipment to ensure materials are available to residents and staff on all shifts
- Monitor changes in resident condition that might require nursing intervention. Report to the resident care manager immediately
- Encourage and assist residents to and from activities.
- Organize and facilitate resident outings into the community as determined by their interest and abilities.
- Transport residents to various events as needed; help with other transportation when necessary, i.e. driving.
- Schedule programs that include at least two weekends a month.
- Perform other duties as necessary.
