Our Temecula, CA location is looking for a caring and compassionate Licensed Vocational Nurse that has the compacity to work with various patients. The ideal candidate will be responsible for providing strong clinical knowledge in all aspects of Home Health Care to patients while adhering to compliance standards. This candidate should be able to recognize patient needs and prioritize those needs with the needs outlined by physicians.

Responsibilities

• Agency by assisting coworkers when workload permits; provides assistance in a courteous manner and treats others with care and respect.1. Providing services in accordance with agency policies;

2. Preparing clinical and progress notes;

3. Assisting the RN in carrying out plan of care;

4. Assisting client in learning appropriate self care techniques;

5. Assist patient and caregiver in care process, supporting patients efforts toward self sufficiency;

6. Involves patient and caregiver in care process;

7. Observes patient for changes in condition and report out;

8. Prioritizes appropriate teachings and interventions;

9. Utilizes appropriate resources to function effectively in home care setting;

10. Participates in monthly in services;

11. Maintains and upgrades professional knowledge and practice skills;

12. Utilizes case manager as first resource when problems occur in field;
