We are hiring a Case Manager for our Support Services department at our Men’s Campus. The Support Services department at UGMTC provides needs assessments, goal planning, case management, consultation and referral services for men experiencing homelessness, hunger, addiction and mental health struggles.

LSW Case Manager POSITION SUMMARY
This role interfaces with UGMTC programs to provide case management, consultation and referral services for guests and residents. With limited supervision, this role also runs our walk-in sessions and conducts needs assessments and goal planning services, including Coordinated Entry housing assessments, as appropriate. This role is critical in connecting Union Gospel Mission guests and residents to all relevant resources.
LSW Case Manager DUTIES AND RESPONSIBILITIES
• Exercises independent judgment in developing and tracking case management plans; providing consultation and referring to appropriate external providers and human service agencies
• Assists Support Services Supervisor in the implementation of program policy and procedures, measures the effectiveness of services, and recommends improvements consistent with the ethical standards of the profession
• Conducts standardized Coordinated Entry housing assessments and maintains timely documentation in Ramsey County’s Coordinated Entry system
• Runs UMGTC walk-in sessions for guests and residents in emergency beds, pay-to-stay beds, and UGMTC housing programs, providing resources and referrals as appropriate
• Uses established case management methodology and practices to establish and monitor resident goals and case management plans ensuring resident confidentiality
• Collaborates with Program Directors, Program Chaplains and Mental Health Counselors on case issues and resident progress, to support spiritual development, coordinate care for residents, and make program recommendations.
• Assists with coordinating professional volunteers in Support Services.
• Builds relationships with key community stakeholders
• Maintains cooperative relationships and communication with other departments to ensure access and positive experiences for UGMTC residents and support the achievement of program objectives.

LOCATION On-site in Saint Paul, MN 55130; Must be able to reliably commute or plan to relocate before starting employment.

LSW Case Manager MINIMUM JOB REQUIREMENTS
• Bachelor's degree in social work
• Professional licensure: Licensed Social Worker or higher
• 2 years case management and social work experience preferred
• Experience working with individuals experiencing mental health issues, homelessness, poverty, addiction or brain injury
• Knowledge of County, Department of Human Services, Ramsey County Coordinated Entry and St. Paul permanent-supportive or permanent housing resources preferred
• Proficiency in a second language (Hmong, American Sign Language or Spanish) preferred
• Valid driver’s license, insurability and ability to transport clients to community resources preferred

• U.S. work authorization (Required)
WORKING CONDITIONS & PHYSICAL EFFORT:
• Work is performed in an emergency shelter/transitional housing environment and involves physical effort encountered in normal, everyday office activities.
• Must be able to lift 20 lbs.
