Must have Social Worker licensed from the State of Texas
What You Will Do: As the Licensed Social Worker, Case Manager, you'll coordinate comprehensive plan of care for patients.
- Oversee primary needs of patient, including assessment of client and family/caregivers, and develop comprehensive plan of care.
- Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician.
- Monitor patient care through assessments, evaluations and/or patient records.
- Advocate for resources and removal of barriers.
- Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
Licensure / Certification / Registration:
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Licensure required relevant to state in which work is performed.
- Counselor preferred. Licensure required relevant to state in which work is performed.
- Social Worker preferred. Licensure required relevant to state in which work is performed.
- BLS Provider. American Heart Association or American Red Cross preferred.
Education:
- High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences:
- 3 years of licensed experience in manage care, insurance, health-care or related field preferred.
- Must have excellent communication skills and working with the vulnerable population.
- Bilingual, preferred.
- Social Worker licensed in the State of Texas, required.
