Qualifications:
A minimum of two (2) years of clinical nursing experience. Experience must be in Hospice or in a medical/surgical setting, a hospital, oncology or geriatric facility. A current LPN license in the State of Florida or Alabama as applicable depending on assignment. Experience must include clerical as well as computer proficiency. Must possess excellent communication and interpersonal skills. Must be able to handle confidential matters. Reliable transportation, valid state of Florida or Alabama driver's license, as applicable depending on assignment, and current automobile liability insurance and insurability under the company liability insurance are required. Must possess current CPR certification.
Main Duties and Responsibilities
Responsible for providing nursing services to meet the personal, physical, and emotional needs of the patient/family unit in the absence of the primary nurse.
  • Uses nursing assessment skills and appropriate questions to accurately evaluate problems/needs.

  • Completes assessment of patient/family unit during visits.
  • Performs treatments/interventions in accordance with the Plan of Care and physician's orders.
  • Provides emotional support to patient/caregiver.
  • Teaches patient/caregiver as appropriate to the Plan of Care.
  • Makes visit at time of death as directed and completes appropriate paperwork.

  • Follows Standard Precautions.
  • Makes follow up phone calls to patients and caregivers within 24 hours when the patient has been experiencing pain, any change or problem, and/or when recommendations were made by the primary nurse.
  • Arranges for needed medications and supplies.
  • Attends Monday morning report.
  • Participates in bi-weekly IDG's.

Responsible for providing Performance Improvement support.
  • Assists with updating of PIF's and Medication/Treatment records.
  • Recopy PIFs when needed. Write Doctor's Confirmation order with all new orders.
  • Call Pharmacia with new medication orders and fax new medications and updates including fax deaths, revocations, and discharges to Pharmacia as appropriate.

  • Monitor patients with increased pain levels and the 24 follow-up for pain control to comply with our guidelines.
  • Assist in recording pain levels on the Pain Graph.
  • Complete Active Patient phone surveys within 3-5 days after a patient is admitted and assist with follow-up with any concerns expressed by patient, patient family, or caregiver.
  • Assist the Clinical Managers with the monthly audits of patients chart for appropriate documentation and continued hospice appropriateness as required by the P.I./U.R. dept.
  • This will include the required 150 day chart review.

  • Assist in the review of all clinical documentation prior to its being placed in the patient's medical record to identify documentation concerns and to secure the appropriate documentation as indicated.
  • Attend IDG meetings.
  • Assist in the review of the IDG Care Plans following each IDG.
  • When a patient is a facility patient, ascertain that a copy of the facility care plan is obtained and on file in our office.

Responsible for providing Intake/Referral support
  • Take referral information per intake form and make appropriate distribution to team members.
  • Make RN assignments with supervision of Admissions Clinical Manager within time frame of Hospice Policies and Procedures.
  • Obtain information on all DME, pharmacy accounts, infusion therapy orders and medical supply requests, and forward to Team Administrative Assistant/Medical Supply Manager.
  • Order and coordinate all infusion therapy orders for pumps and needs.

  • Assign Admissions numbers.
  • Field information calls regarding Hospice and send info packets as appropriate.
  • Perform routine pre-admission visits to the patient/family unit at planned intervals, contributing to the ongoing updating and maintenance of admission plans.
  • Communicate on a regular basis to Admission team members or to on-call nurse the current status of assigned patient/family unit.

Coordinates appropriate care, communicates and collaborates with Interdisciplinary Group.
  • Notifies team manager and team members by leaving accurate and detailed messages in voice mailboxes prior to going off duty.
  • Leaves message as per policy after death attendance.
  • Accurately completes documentation of visits and phone contacts, and submits documentation in a timely manner per policy.
  • Calls physician as necessary for changes in Plan of Care for each patient and updates twice monthly or more often if needed.

  • Initiates continuous care when needed as per policy.
  • Follows policy if Inpatient care is required. Develops Hospice Aide plan of care based on patient's personal care needs.
  • Makes supervisory visits to patients being cared for by a Hospice Aide.

Actively seeks ways to develop professionally.
  • Develops basic knowledge of Hospice Medicare/Hospice Medicaid and other reimbursement mechanisms.
  • Develops areas of expertise in hospice nursing skills.
  • Pursues professional growth through academic and continuing education programs.
  • Attends Hospice or Community sponsored in-services. Attends all mandatory in-services.

Competently performs all basic nursing skills (see attached "New Employee Nursing Skills List").
Completes Clinical Competency orientation within the 90-day probation period.
Determines the patient is hospice appropriate, documents to the patient's terminality, and establishes and maintains the care plan for medically necessary assessments and services. This applies to determinations and consultations with members of the IDG, including the Attending Physician or Hospice physician, in all circumstances in which the level of care is to be assigned as Short-term Inpatient Care or Continuous Care, and includes all actions required in Covenant's policies and procedures regarding the placement or removal of a patient from those categories of care.
As a member of the Interdisciplinary Group, is responsible for determining and documenting the level of care status of patients, and in particular, inpatient level of care, to ensure the patient is appropriate and meets the criteria for short-term care needed to manage pain and symptoms in an inpatient facility. This responsibility includes appropriate management and documentation with the patient's change in status either to short-term inpatient care or removed from that status, and all monitoring of the care during the inpatient period. This same requirement applies to management and documentation of patients who are placed on a Continuous Care level of care. Strict compliance with all rules and regulations, including Covenant Hospice's policies and procedures, is required.

  • Maintains a good attendance record.
  • Notifies agency of absences according to policy.

  • Keeps neat and legible written documents.
  • Participates in agency Q/A as appropriate.
  • Attends mandatory general staff meetings, and other meetings as requested by supervisor.

Required to comply with all Covenant Hospice Policies, Procedures and Personnel Policies, including those specifically addressing disaster preparedness and response.
Works effectively with volunteer services to identify patient/family needs that can be met by volunteers; offers volunteer services to patients and families, as needed, throughout their length of stay; requests volunteer services to meet identified needs; documents volunteer interventions in the Plan of Care.
Special Demands/Physical Requirements
Position frequently requires medium to heavy lifting. Normally, such lifting will include, but is not limited to, assisting with transfer/repositioning of the patient either in the home or nursing facility. This position is also mentally demanding because of the continual need of assessing and evaluating certain clinical aspects of the patient's care. This position requires considerable time traveling in a motor vehicle in connection with performing the essential functions of the position.
The position does require the mental ability to analyze data, covert data to useful information develop and present briefings and interact successfully with a diverse number of individuals
