Licensed Practical Nurse Referrals Assistant (Rotating Weekends)

Job Description

Under supervision, meets or communicates by telephone, e-mail, or fax with key referral sources in a consultative manner regarding their home care needs and present appropriate solutions. Aids the paper flow of new referrals to ensure timely provision of services.

Job Responsibility

• Assists in processing referrals for one or more home care product lines.
• Aids the completion of enrollment documentation.
• Initiates insurance verification form.
• Keeps referral logs on daily basis.
• Maintains appropriate files and records.
• Maintains current knowledge of regulatory, reimbursement and third party contract guidelines.
• Documents pertinent referral information within established timeframes and payer guidelines.

Job Qualification

• Vocational or Technical school Diploma required, or equivalent combination of education and related experience.
• Current License to practice as a Practical Nurse in New York State, and Current Basic Life Support Professional Certification required, plus specialized certifications as needed.
