Reports to: Clinical Development Manager
Rate of pay: $18.72/hour - $21.69/hour
Work schedule: Monday - Friday, primarily day shift; the day-to-day schedule may vary slightly based on the needs of the Health Center; no evenings, no weekends, no major holidays
POSITION SUMMARY:The Licensed Practical Nurse (LPN) shall carry out direct nursing care to patients of the health center under the supervision of a physician or registered nurse.
PRIMARY RESPONSIBILITIES:
- Adheres to statutory and regulatory requirements in regard to certification and quality of health care as well as agency-approved policies and procedures.
- Interviews patients, obtains vital signs, prepares patients for exams, and assists providers with exams and/or treatments.
- Updates and ensures the accuracy of patients' electronic medical records.
- Take patient telephone calls to identify emergent and non-emergent appointment criteria.
- Oversees telephone prescriptions and refills to pharmacies.
- Maintains coordination of services with individuals and/or qualified persons.
- Coordinates with vendors to ensure delivery of prescribed equipment as needed.
- Cooperates with providers and personnel to ensure quality programs, regular reviews, and accurate data collection.
- Assists with ordering and maintaining medical supplies and equipment.
- Acts as a member of the team, assuring services are appropriate to meet the needs of each patient.
- Advises supervisor of unmet needs of the health center to help expand services for patients.
- Maintains standard of infection control.
- Other responsibilities assigned by the supervisor.
- Licensed Practical Nursing Certification.
- Experience in community nursing is preferred.
- Current BLS Certification
- Proficient oral and written communication skills.
- Strong organizational and planning skills.
- Ability to maintain good working relationships with others.
- Exhibits a high level of professionalism and confidentiality in dealing with issues.
- Excellent computer skills. Electronic medical record experience preferred.
- Ability to lift and carry up to 25 lbs., lift and support patients with and without help.
- Ability to walk and stand for extended periods.
- Regular exposure to infectious diseases, blood and airborne pathogens, needle sticks, hygiene issues, and other odors.
- Able to tolerate clinical smells.
- Ability to read digital readouts, recognize colors, and operate necessary medical equipment.
- Ability to operate mechanical devices used by patients.
- Ability to wear necessary hospital masks, gowns, gloves, and close-toed shoes.
