We are seeking a Licensed Practical Nurse Lead to join our7th Ave Outpatient Clinic. This location provides mental health treatments, including medication monitoring and assistance for persons with addictions.
The Licensed Practical Nurse will work in-office Monday-Friday, standard business hours (no evenings, weekends or holidays).

The Licensed Practical Nurse serves as a central support within the interdisciplinary sites team. This staff member is responsible for providing methadone-dosing services to appropriate clients.
Responsibilities:
- Completes blood draws and sends blood to the lab
- Monitor known health conditions such as diabetes, high blood pressure, and obesity
- Performs injections
- Review the effects and side effects of medications
- Provide assessment and monitoring of physical/medical status
- Accepts medication orders from physician via telephone and medication records
- Participate in Clinical case staffing
- Facilitate coordination with the consumer's clinical team and PCP
- Provides accurate documentation of contact with members
Qualifications:
- Licensed in the State of Arizona as an RN
- Ability to obtain fingerprint clearance card
