Memory Care Program Manager / Director

The Terrace is a classic Skilled Nursing Facility that has 115 beds, with a heavy emphasis on long term resident care and a small, but growing, short-term rehab unit. This close-knit team is welcoming and always willing to jump in to help out when you need it. What makes us unique:

As a Memory Care Program Manager, you will implement, lead and manage all functions for this interdisciplinary, facility wide program. We do not have a locked Dementia/Memory Care Unit; our patients are throughout the center. You will hire, train and supervise your team (activities and support staff). You will be involved with quality measures, budgeting, events calendar, program management and reporting metrics. You will partner with our strong therapy director and be supported by a dynamic regional leader ready to help you succeed. This is a highly visible and hands-on leadership position, where you will be interfacing with patients, family members and staff every day.

Sometimes this position is referred to as a center's Alzheimer's Unit Manager or Memory Care Director. We call it a Program Manager.
Here's what we need from you:
- * Ideally, 1-2+ years prior experience in a skilled nursing facility as a Memory Care Unit/Program Manager (or similar title)
- * Prior experience as an Activity Director is desired (you will be overseeing the Activities team/dept)
- * Certified Dementia Care Practitioner or a Certified Dementia Care Trainer is helpful
- * Assoc. or BA degree in a health related field is ideal. Such as: or a Licensed Practical Nurse (LPN), Social Worker (or similar), 36 hour IL Activity Director Certification...
- * Electronic medical records experience (Point Click Care or PCC) is ideal
