PRIDE Health is hiring a Clinic LPN in Clermont, FL! (contract)
Type: 4 months
Schedule: M- F (Day shift)
Pay Rate: 30-33/ hour
Job Responsibilities:
- Case Management
- Clinical Support to Medical Assistants
- Working with Providers on high risk patients
- Daily, Weekly, Monthly follow up of High Risk patients
- Coordinating all post discharge, ER visit plan of care
Qualifications:
- BLS (AHA) or CPR/AED for the Professional Rescuer (American Red Cross) or the ability to obtain within 90 days of employment
- LPN Certification for Florida
- Case Management experience
