Description of Job
This is a regular, full-time, in-person, essential services designated position. LPN Nurse licensure is required to work within this 24/7 facility. Shift work is also mandatory and includes day, evening, and night shifts to include holidays and weekends as needed.
Please note: These working arrangements are subject to change.
The purpose of this work unit is to provide basic nursing care and provide a skilled level of nursing to residents. Reviews and audits specific care activities to determine compliance with policies and standards of care as part of a development quality assurance activity. Administers specialized and routine nursing services such as medication and treatments. Documents care provided in the medical record and other appropriate documents. Evaluates residents upon admission and throughout the
clinical stay when critical incidents, emergencies, change of condition, or acute illness and behavior changes to identify nursing interventions. Administer IV fluids (if IV certified) as prescribed by a physician. Assess and evaluate the effectiveness of the care/treatment given.
Primary Job Duties
Standard Assessment:
  • Performs both admission and daily assessments during clinical stay, identifying critical stay,
    identifying critical incidents, emergencies, acute illnesses, and behavior changes with RN oversight.
  • Identifies and implements appropriate nursing interventions, treatment care plans and physician referrals.
  • Documents current, accurate, and pertinent information regarding care and treatment provided to individual residents.
  • Indicates specific changes in status, medications, and physician orders.
  • Detects abnormality in resident's health status indicated by symptoms, vital signs, and laboratory results and reports information to physician.
  • This position is required to drive a State vehicle to do off-site pre-admission assessments or take a resident to the doctor or the hospital (if not taken by ambulance).
  • Must have an RN co-sign admission assessments.
Regulatory Compliance
  • Directs and implements nursing care to residents by assuring compliance with accepted
    standards of nursing care, departmental and facility policies and procedures, and VA and State of Colorado regulatory requirements.
  • Directs subordinates to ensure these standards are met. Reviews and audits physician orders, MARS, CNA flow sheets, bathing schedules, and BM sheets to determine compliance with policies and standards of care as part of a department quality assurance activity.
Medication and Treatment Administration
  • Competently administers medications to residents within the scope of practice while observing
    the five rights of safe medication administration, resident rights, infection control standards, and appropriate nursing policies.
  • Documents all medications that are given and justification for scheduled medications not given.
  • Appropriately identifies changes in resident's conditions that need to be assessed by a registered nurse.
  • Identifies problems with medication administration, interactions, reactions, and compliance.
  • Participates in medication/pharmacy work teams to ensure quality improvement and optimal practice.
  • Communicate concerns to appropriate staff to prevent further complications or to ensure timely appropriate intervention.
Safety Identification and Intervention
  • Monitors the clinical, physical, and environmental safety of all residents through assessments and regularly scheduled rounds.
  • Identifies and implements appropriate clinical, physical, and environmental interventions, performs necessary documentation for regulatory compliance, and makes appropriate notification to family, physician, NHA, and DON.
Training, Resource Team Member and Delegation
  • Serves as a resource to subordinate staff to ensure timely and proper care of residents. Trains subordinate staff and assures competencies are completed and submitted.
  • Provides input to management on the direct care performance, documentation, attendance, and interpersonal skills of subordinates for the purpose of evaluation.
  • Evaluate daily workload and prioritize assignments with available staff.
  • Delegates duties to provide the most effective coverage to ensure quality and continuity of care.
  • Maintain updated skills, knowledge, and competencies necessary to the position through continuing education, clinical supervisory literature review, and participating in discipline and team development.
Communication
  • Provide active listening and communication, support, and encouragement to residents and families regarding their concerns by both phone and personal contact.
Other Duties as Assigned
  • Performs other related duties as assigned.

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights
Minimum Qualifications:
Current, valid licensure as a LPN from the Colorado Board of Nursing or any state participating in
the Enhanced Nurse Licensure Compact (eNLC).
**Complete Board approved IV certification course including supervised clinical; OR document
completion of such from another state Maintains evidence (both LPN and employer) of original documentation and demonstration of IV knowledge/judgment/skill Follows written IV policies and
procedures approved by medical staff, nursing, and agency administration
**Advanced Educational Preparation for LPNs in Colorado is limited to IV certification.
AND
Three years of professional experience in an occupational field related to the work assigned to the position.
**Note, nothing in these Minimum Qualifications alleviates the criteria to transfer licensing or any requirements by the respective board under the Colorado Department of Regulatory Agencies (DORA).
Substitutions:
There is no substitution for the required license.
Appropriate education will substitute for up to two years of the required experience on a year-for-year basis.
Conditions of Employment:
  • Must complete a full background check to include, CBI name check and fingerprint check, ICON Colorado court database, Medicare fraud database, Reference Checks, Professional License verification (licensure requirements), Drug Screen, CAPS (direct contact with adults - Mental Health Institutes, Regional Centers, Veterans Community Living Centers).
  • Must possess a valid US Driver's License.
  • Travel: May have to transport residents in state vehicles.
  • Must be a licensed LPN from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact in good standing. Qualification cannot be obtained during the probationary/trial service period.
  • Must acquire and maintain a current CPR Card and perform CPR adequate compressions and ventilations per CPR guidelines.
  • Pre-employment Physical/Tests: TB test-Vaccinations as required by state and facilities
  • "Essential Service Designation": you will be required to report without delay or interruption to provide essential or emergency services to ensure health, safety, and welfare.
  • This is a 24/7 facility. Must be able to work any and all shifts: day, evening, and night shifts to include holidays and weekends as needed.
  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation as to why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.

*Sign-On Incentive:
Upon approval, a sign-on incentive based on position, ranging from $2,000 to $7,000 (prorated if a pa
rt-time position), will be given in three payments: within two pay cycles of when the new employee starts at CDHS, 180-day anniversary and 270-day anniversary with CDHS. These incentives will be available to the first 1,000 new employees in select high-need positions.
