Nurse Qualifications:
- Med-surg experience
- Open to candidates new to Home Health
- New graduates are welcome
- Active LPN license in the practicing state
- Current CPR certification
- Willingness to travel to patients' homes
Nurse Responsibilities:
- Provide individualized care in a home setting
- Deliver specialized nursing care to complex adult patients
- Conduct patient assessments and coordinate care plans
- Offer health education and training to patients and families
- Availability to work one weekend per month
