Are you the One? We are looking for a Licensed Practical Nurse II, Non-Institutional,who will work under the general supervision of the Area Administrator and Registered Nurse Supervisor. Maintains knowledge of Community Long Term Care programs and related systems including Phoenix. Conducts pre-admission assessments for persons seeking Medicaid sponsored nursing home placement and for persons seeking admission to Community Choices Waiver and Home Again. Completes PASRR Level I and II activities for those applicants seeking Medicaid sponsored Nursing Home Placement.
- Performs Pre-admission Assessments for Community Choices waiver, Home Again Program and Nursing Home Placement.
- Performs PASRR Activities.
- Provides CLTC Education and Referral.
- Attends Program Related Training.
- Performs emergency preparedness duties or activities as directed.
- Performs Other Duties As Assigned.

Requires current, active and non-restricted licensure as a Licensed Practical Nurse by the State Board of Nursing for South Carolina.
A minimum of three (3) years of clinical experience in an acute or med-surgical environment and two (2) years of healthcare experience that demonstrates expertise in conducting utilization reviews and/or Prior Authorization or related experience.
NECESSARY SPECIAL REQUIREMENT: Current licensure as a Licensed Practical Nurse by the State Board of Nursing for South Carolina.

- Sitting or standing for long periods of time.
- Effective time management skills and computer proficiency.
- Must be able to perform web-based software functions(key data, run reports, update documents, navigate webinars etc.).
- Considerable knowledge of social, mental and cultural aspects of illness.
- Written and verbal skills in assessing and evaluating applicants' conditions and needs and in conversing with applicants, family members and other community contacts. Knowledge of HIPAA regulations.
