The Licensed Practical Nurse (LPN) is responsible to provide nursing duties at the Behavioral Health clinic and will be required to gather Health Information, ordering of medications, provide updates to the RN and Behavioral Health care providers, assist the practitioner as necessary, schedule appointments, make referrals, complete required documentation and participate in Agency activities.
Core Responsibilities
- Monitor Providers daily schedule and assist in ISTOP documentation activities
- Provide daily nursing care to patients in the behavioral health care clinic.
- Assist the Providers by accessing RHIO.
- Ensure that lab work is obtained.
- Monitor incoming lab, consultation reports and telephone calls to ensure that Providers are updated.
- Ensure that prescriptions are sent into pharmacy as ordered.
- Monitor all medications onsite.
- Complete all required documentation and reports according to program regulations including program participants' charts.
- Ensure compliance with all pertinent government and agency regulations and operating standards
Qualifications
- A.A.S. Degree or High School Degree or equivalent plus Specialized Trade Certificate.
- Current NY State LPN license and registration.
- Experience working with the substance abuse population preferred.
- 1-3 years' experience.
- Valid NYS Drivers License.
