The Licensed Practical Nurse I is knowledgeable about and supportive of Patient and Family Centered Care Model using a collaborative approach with the Register Nurse. The LPN I demonstrates clinical judgment by applying problem-solving techniques and by integrating skills and knowledge into clinical practice. 1. Provides direct patient care with the establish model of nursing as evidenced by the following:
1.1 Documents admission information under supervision of the Register Nurse
2. Collaborates with the Register Nurse and other members of Health Care Team.
2.1 Collaborates consistently with Register Nurse to develop the patient-s plan of care and implement interventions on all assigned patients.
3. Verifies, prepares and administers medication to all assigned patients to ensure safe medication delivery and positive outcomes.
3.1 Clarifies any questions regarding physician prescriptions orders immediately to the Register Nurse 100% of the time.
4. Participates in team building activities that contribute to the patient care, staff education, and self development.
4.1 Participates in interdisciplinary conferences
5. Maintains clinical competency to ensure that practice is evidence- based.
5.1 Attends all mandatory and continuing educational classes.

Additional Information:
Ability to work in a very fast paced multispecialty clinic/infusion center. 2 years of ambulatory experience. Proficient with EPIC.
