`Duties```
- Provide direct patient care, including administering medication, monitoring vital signs, and assisting with activities of daily living.
- Perform assessments and document patient conditions and progress.
- Collaborate with the healthcare team to develop and implement individualized care plans.
- Educate patients and their families on disease management, treatment options, and self-care techniques.
- Assist with pain management techniques and provide emotional support to patients.
- Administer treatments and procedures such as IV insertion, inhalation therapy, and wound care.
- Utilize electronic medical record systems such as Cerner and ICD-9 coding for accurate documentation.

- Valid LPN license in the state of [Location].
- Previous experience in nursing, preferably in a healthcare or long-term care setting.
- Knowledge of dementia care principles and techniques.
- Proficiency in medication administration and dosage calculations.
