Okalee of Medina is seeking a friendly and organized Full-Time orPart-Time LPN to join our team.
JOB DUTIES/RESPONSIBILITIES:
1. Coordinates resident care, conducting meeting with staff to address resident care issues.
2. Ensures that unlicensed staff duties and responsibilities are accomplished in a timely, quality manner, and reporting problems to RN.
3. Report changes or problem in resident condition to RN.
4. Initiate emergency treatment and report to RN Supervisor.
5. Participate in the completion and review of nursing care plans.
6. Assist with administration of physician orders.
7. Carry out proper charting practices and procedures.
8. Consult with RN about resident conditions and their treatments.
9. Assist with new admissions, hospital transfers and discharges as requested by RN supervisor.
10. Order and administer medications.
11. Attend required meetings, in-service programs, and conferences.
12. Work cooperatively with other departments.
13. Performs other duties as assigned by RN supervisor.
14. Assist in the monitoring and evaluation of job performance for unlicensed staff.
15. Consults with RN regarding unlicensed staff performance problems.
16. Train new employees entering the department as directed by supervisor.
17. Plan, assign and direct work of unlicensed staff within established guidelines.
18. Assist unlicensed staff as necessary.
19. For resident and employee safety reasons, you must be able to lift 50 lbs. unassisted, at all times during your shift.
20. This facility operates 7 days a week, 24 hours a day. Schedules must accommodate fluctuating facility demands and team members may be asked to work shifts other than those they prefer or normally work.
21. As staffing needs dictate, mandate (per state law) will be required, and you must be able to work beyond your regular shift hours of which may mean more than 40 hours for that particular week.
22. First on call rotation scheduled for staff to have access to a professional nurse.
23. Rotating weekends and holidays.

- Personal/business conversations among team members to be stopped in order to assist residents.
- All team members should take the initiative to offer assistance or answer questions throughout the community.

WORK ENVIRONMENT:
¨ Primary responsibilities will be conducted in clean, well-lit, climate controlled areas such as nurse’s office, resident apartments, hallways, work rooms, resident dining area and meeting rooms.
- The noise is usually moderate.
- Exposure to bloodborne pathogens and infectious diseases.
- Exposure to chemical and respiratory hazards.
- Some travel to meetings and seminars may be required.
PHYSICAL DEMANDS:
- Work hours as assigned, scheduled, or mutually agreed upon.
- While performing the duties of this job, employee is regularly required to use hands and fingers to handle or feel objects, tools or controls, and reach with hands and arms. The employee frequently is required to stand and walk. Occasionally required to sit, climb or balance, stoop, kneel, crouch, talk or hear, and taste or smell.
- The employee must frequently lift and/or move up to 10 pounds and regularly lift and/or move up to 50 pounds. Occasionally lift and/or move up to 100 pounds. Push up to 75 pounds. Specific vision abilities include close vision, distant vision, color vision, peripheral vision, depth perception and the ability to adjust focus.
POSITION REQUIREMENTS/QUALIFICATIONS:
- To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
- Must be familiar with general nursing and geriatric nursing practices.
- Must demonstrate leadership, good judgment and dependability.
- Must be knowledgeable and accurate in the administration of medications and treatments.
- Must possess good listening, verbal and written communication skills.
- Must be able to perform duties of unlicensed staff.
- Must possess a desire to work with elderly and handicapped persons.
- Good organizational and planning skills.
- Must be able to train and teach procedures, policies, etc.
- Must work as a team member and maintain and positive demeanor.
EDUCATION AND/OR EXPERIENCE:
Graduate of accredited school of nursing for Licensed Practical Nurse.
CERTIFICATES/LICENSES/REGISTRATIONS:
Must be a Licensed Practical Nurse within the State of Minnesota, and maintain licensure.
