ResponsibilitiesThe Licensed Physical Therapist provides coordinated care to patients of all age groups. Plans, implements, and evaluates patient care plans to restore or maintain patient well-being. Plans, organizes, and conducts physical therapy treatment program based on the medical referral and their evaluation. Participates in the coordination of care.
- Performs ongoing clinical assessment, including OASIS assessments, at appropriate time points to identify patient needs and determine level of functioning and appropriateness of patient for home care.
- Develops and evaluates the plan of care in partnership with the physician, patient, representative (if any), and caregiver(s). Revises the POC based on evaluations and ongoing assessment data.
- Coordinates care and documents coordination with all physicians and other health care clinicians involved in the care of the patient as needed and appropriate.
- Provides patient caregivers and family education.
- Documents, prepares, and completes clinical notes, phone orders and other clinical record documentation on an ongoing basis and in a timely manner.
- Provides treatment and services according to state and federal regulations and professional standards.
- Applies concepts of infection control and standard precautions in coordinating or performing patient care activities to prevent contamination and transmission of disease.
- Provides supervision to Physical Therapy Assistants and Aides, when applicable, in the performance of patient care that is consistent with the patient's needs.
- Meets mandatory continuing education requirements of the Agency and licensing board.

- Participates in the Agency's QAPI program.
- Participates in Agency-sponsored in-service training.
Education: Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
Licensure: Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC, to practice Physical Therapy, or if licensure does not apply, meets one of the requirements listed in 42 CFR 484.115(h)
Experience: Prefer one year experience as a Physical Therapist in an acute care, rehabilitation setting or home health.
Skills: Therapy skills as defined as generally accepted standards of practice, good interpersonal skills, proof of current CPR, knowledge of durable medical equipment
Transportation: Reliable transportation, current Driver's License, valid and current auto liability insurance
