Responsibilities
- Performs duties within scope of license and within care plans.
- Works closely with care team on providing exceptional care and services; provides adequate communication to the team responsible for the patient.
- Assists patients by providing personal services, such as feeding, bathing, dressing and grooming.
- May assist in light housekeeping or laundry.
- Monitors patient condition by observing physical and mental condition, intake and output and exercise.
Preferred Qualifications
- Positive experience with complex patients with dementia, Alzheimer’s disease, COPD, cancer and joint/hip replacements.
- Ability to perform PT/INRs.
Required
- Registered as Licensed Nursing Assistant with New Hampshire’s Board of Nursing.
- CPR Certification.
- Satisfactory criminal record.
