Principal Responsibility:
About the St. Anthony's Companionship Program:
This peer mentorship program provides support to individuals in recovery and/or people experiencing homelessness. Drawn from more than seven decades of best practice and evidence-based research, the Companionship Program is a scalable, multi-year approach tailored to each person to break the devastating cycle of addiction, homelessness, or poverty. Over a 3-5 year period, Companions work with participants to design and achieve their own personal goals. Companions provide non-clinical and long-term wellness support through training, mentoring, and coaching in ten areas necessary for stability: substance use recovery, health, finances, food and housing, work, and wages, resolving or complying with legal issues, family and social networks. We currently serve 40 participants in the program, and the goal is to provide companionship services to hundreds of guests within the next few years. The program initially began supporting participants at our men's recovery program, Fr. Alfred Center, and is currently expanding to serve guests (both men and women) in St. Anthony's other programs as well as participants at partner organizations.
This Role
The St. Anthony's Companionship Program Manager (CPM) leads the implementation and expansion of the Companionship program, overseeing a growing team that provides ongoing, multi-year support to St. Anthony's guests with a goal of improving their stability and helping them sustainably exit homelessness or poverty. Reporting to the Director of Clinical Services at St. Anthony Medical Clinic (SAMC), the CPM will manage the companion program with a comprehensive therapeutic approach and professional behavioral guidance. Responsibilities include both quality assurance and direct client care, ensuring all services are in compliance with county, state and federal regulatory agencies, industry best practices and the core values and mission of St. Anthony Foundation. The CPM will liaise with a care team that includes medical providers, support staff, health educators and other St. Anthony programs, as well as external partners.

  • Provides recommendations and assists in long-range strategic planning for the program.
  • Identifies target participant groups and develops appropriate interventions to help them achieve more stability in their lives, aligned with the St. Anthony's stability framework.
  • Evaluates program development and implements continuous improvement methods to adapt and provide the best quality care in collaboration and with guidance from the Director of Clinical Services.
  • Oversees internal and external program partnerships to build program capacity to meet guest's needs.
  • Creates effective referral processes and tracks and measures their effectiveness.
  • Develops and updates Companionship policies and procedures, incorporating evidence-based best practices.
  • Responsible for program compliance with all HIPPA, data and documentation requirements as identified by county, state, and federal regulatory agencies and SAF policies.
  • Collaborates, communicates and coordinates across departments when necessary without compromising ethical or clinical guidelines.
  • Works with the Learning & Evaluation team to define and measure program outcomes, with the goal of measuring success for participants as well as enhancing program effectiveness.
  • Sets and follows high standard for data and impact evaluation, and designs processes to regularly, efficiently and accurately capture necessary data.
  • Ensures the Companion program operates in accordance with St. Anthony Foundation goals and the SAF Statement of Values.

Team Leadership
  • Comprehensive oversight of the Companion team including consistent evaluation of performance, guidance with situational issues, growth through development and training, and organization of resources.
  • Develops companion operating procedures and ensures compliance in alignment with behavioral health ethical standards.
  • Meets weekly with the companions to guide, support and exchange information to ensure consistency and accuracy of recommended treatment and treatment approaches.
  • Work in an integrated setting offering collaborative care with primary care teams via consultation, shared patient visits, warm hand-offs or other forms of patient care integration activities.
  • Facilitate the exchange of information (within legal parameters) with other health and/or rehabilitative agencies to provide continuity of care for clients receiving services from more than one agency.

MINIMUM QUALIFICATIONS:
  • Licensure in the State of California as an MFT, LCSW or doctoral level psychologist
  • Fluency in English required and Spanish desired. Bilingual in Cantonese also helpful.
  • 3-5 years' counseling or case management experience desired with proven expertise in assessing and managing the psychosocial needs of diverse, low-income, and medically underserved populations.
  • Extensive knowledge of AOD and co-occurring disorder best practices. Knowledge of interrelationship between chronic illness and mental health including addictions.
  • Knowledge of and experience with crisis intervention theory, cognitive behavioral treatment, short-term treatment strategies and practice techniques.
  • Ability to integrate therapeutic mental health services with services provided by other disciplines, especially primary care.
  • Ability to assess, advocate for and provide interventions and treatment to clients in an individual, family or group setting.
  • Ability to work in a medical environment as a member of an interdisciplinary team including addiction treatment and a medication assisted treatment (MAT) program.
  • Experience in the assessment and treatment of psychological trauma.

  • Team-building skills, including experience developing, leading, and supervising multiple level employees in various roles.
  • Ability to handle emergency and high-pressure situations.
  • Excellent interpersonal, listening, oral and written communication and conflict de-escalation and resolution skills.
  • Demonstrated ability to exercise sound independent judgment and effectively manage time and resources.
  • Demonstrated experience in developing and managing budgets and resources.
  • Familiarity with or skills necessary to use electronic medical record software.
  • Knowledge of applicable mental health laws and ethical standards.
  • Commitment to St. Anthony Foundation Statement of Values and a desire to work for a social service agency serving the poor.
