Heritage New York Medical, PC (HNYMPC) is a medical practice with an interdisciplinary team of Physicians, Nurse Practitioners, Clinical Pharmacists, Licensed Clinical Social Workers, RN Care Managers and highly skilled support staff to bring the best care to patients. The team is clinically supervised and supported by two physicians to develop and regularly update patient treatment plans; discuss treatment plan options with patients and their families; and manage patient discharges.
HNYMPC is seeking Licensed Clinical Social Workers to focus on assessment, diagnosis, treatment and prevention of behavioral conditions, and management of stressful medical, social & emotional difficulties. HNYMPC primarily serves the geriatric patient population in a managed care environment. The LCSWs participate in chronic disease management, including psychosocial assessments and population-based education to identify emotional, social, and environmental strengths and problems related to their illness, treatment, and or life situations.
We look forward to a dynamic LCSW to join our team to facilitate the delivery of interventional social services, assist patients with aging in place in their communities, and provide emotional support to patients in various settings. The ideal candidate for this position will coordinate the care of older patients across settings, including patient homes, PCP offices, and via Telemedicine. The focus includes identifying any social determinants of health and overcoming these barriers, discharge planning with the interdisciplinary team, and psychosocial assessment and treatment.
Responsibilities include:
- Conduct a comprehensive psychosocial assessment of the patients’ presenting issues, establishing a patient-centered care plan aimed at resolving identified issues. This will include any relevant social history.
· Determine appropriate milestones to gauge client progress
- Maintain a caseload via phone communications, home/office visits and/or telemedicine live video appointments daily
- Utilize brief, solution-focused interventions to resolve the clients’ presenting issue
- Ensure safety for clients who present at risk to harm self/others
· Communicate client updates to all relevant parties as needed (Families and Physicians, etc.)
· Facilitate referrals through the patient’s health plan, to other healthcare professionals and/or community resources/programs
- Maintain up-to-date documentation for all patient contacts in the appropriate electronic medical record system per established procedures and processes
· Maintain accurate client documentation along with submission in a timely manner.
· Performing administrative and record-keeping duties
· Ensure high quality service delivery, including: attending weekly social work staff meetings, internal and external case conferences, client referral and follow-up, scheduled supervision and client meetings as needed.
· Develop and maintain referral linkages with outside agencies and organizations to meet the needs of program patients.
· Escort patients in crisis for emergency intervention.
· Maintain familiarity with both professional and self-help systems in the community to assist the patient in those services when necessary.
- Continually strengthen professional knowledge base through review of trends related to telehealth counseling, effective treatment modalities, and evidence-based interventions
- Other duties as assigned
Work Location: Hybrid position
· Mix of telephonic and telemedicine patient contact on remote days.
· Home visits for 2 days per week
· Weekly office day on-site in our main office.
· Occasional visits in PCP offices.
Job Type: Full-time
Pay: $80,000.00 - $98,000.00 per year
