Description
This position will serve as Program Manager and Behavioral Health (BH) clinician primarily for the Medication Assisted Treatment (MAT) program in Burke County to assist patients with opioid use disorder in office and in the field in the low barrier MAT program. This position will include administrative, case management, and clinical duties.
Responsibilities include:
MAT Administrative
The MAT Program Manager will provide administrative oversight for the MAT program in Burke County. This includes the office-based and low-barrier programs. The Program Manager will conduct team meetings, schedule patients and coordinate care for MAT
patients.
MAT Clinical
MAT provider will conduct substance use specific assessments for OUD for the HCCH MAT office-based and low-barrier program. Provide clinical counseling and case management for patients who are participating in the MAT program, using evidence-based practices including but not limited to motivational interviewing, CBT, relapse prevention, harm reduction and trauma-informed approaches. Collaborate with prescribing medical providers to treat MAT patients based on current best practices and
HCCH policies and procedures. This position will work closely with MAT medical providers, peer support specialists, and
community support, both in the field and in the office.
Licensed Mental Health Provider
The Licensed Mental Health Provider is responsible for providing and coordinating mental health care for patients and their families at HCCH. This clinician provides Behavioral Health assessment and treatment services to referred patients. The clinician
will provide evidenced-based interventions including but not limited to CBT, behavior therapies, solution-focused therapy, and other brief models of treatment. The clinician monitors symptoms and response to treatment (including medication), and works
closely/collaborates with the primary care provider, consulting psychiatrist (as needed), other Behavioral Health consultant(s) and the Patient Resource Specialist to revise the treatment plan when clients are not improving.
In Addition
The provider monitors patients closely for changes in the severity of symptoms and medication side effects, educates patients/families about treatments/effects as needed, encourages treatment adherence, participates in staffing as needed with the inter-disciplinary treatment team documents all encounters into electronic health records according to organizational policies and procedures and best practices, monitors outcomes, facilitates treatment referrals as needed, and completes relapse prevention
plans with clients in remission of symptoms. The provider will work closely with community partners to assist HCCH patients with
receiving psychiatric services as needed, completing initial intake assessments and referrals for medication management as needed.
Requirements
- Licensed mental Health Provider (independent licensure preferred). Applicants must have substance use treatment experience, with LCAS preferred. The candidate will possess expertise in assessing and treating a wide variety of conditions including but not limited to depression, anxiety, trauma, child and adolescent disorders, personality disorders and bipolar disorder.
- Effective written and verbal communication skills. Demonstrated ability to establish rapport quickly with a wide range of people. Ability to work under supervision and collaboratively in an interdisciplinary team setting. Possess a pleasant, open attitude and
a professional awareness of boundaries with patients, co-workers, and supervisors. Seek supervision and consultation as needed with peers and supervisors. Demonstrate an ongoing interest in learning. Competent computer usage. Desire to work with an
ethnically and culturally diverse population. Excellent working knowledge of DSM 5 diagnoses and evidence-based treatments for mental health conditions, and behavioral and relationship problems.
