Job Types: Part-time (Monday, Wednesday, Thursday and Friday)- 30 hours a week
Pay: $24.00 to $29.00
Schedule: 8:30 a.m. to 4:00 p.m.

Job Duties of LPN:
Understands and adheres to established Agency policies and procedures.
Participates in the development of standards, which ensure safe and therapeutically effective service to patient/family.
Consults with the Director of Nursing/Administrator in determining a staffing pattern, which will accomplish stated objectives and promote a maximum level of utilization of health personnel.
Assist in recruiting, hiring, evaluating and terminating nursing personnel. Assist in instituting, interpreting and documenting disciplinary action.
Responsible for orientation of new home health aides.
Assist in planning and consultative needs of staff.
Serves as a role model for all colleagues by setting an example of high standards in dress, conduct, operation and job performances.
Observe confidentiality and safeguard all patient related information.
Serves as a resource person for employees.
Accepts responsibility for regular attendance and punctuality; fulfills job related requirements without regards to time involved.
Assist the Director of Nursing in conducting patient care conferences, staff meetings and maintain documentation.
Immediately reports any accident or incidents to the Director of Nursing/Executive Administrator.
Must be able to perform nursing functions in times of staff shortage.
Available to share on-call with nursing personnel.
Makes staff and patient assignments on on-call.
Implement the nursing home health aide care plan for each assigned patient.
Supervise Home Health Aides.
Participate in medical record audits as assigned.
Other duties as assigned by the Director of Nursing/Administrator..
Ideal candidates will have the following:

• Must have reliable transportation or driver's license with current auto insurance.
• LPN License in OH and CPR.
• Recent and relevant experience (LTC or Facility Staffing).
