• Help individuals achieve a happy and healthy, inclusive life by conducting assessments, implementing health care plans, and monitoring conditions
• Collaborate with physicians and external medical partners who prioritize outcomes as much as you do
• Leverage your clinical skills to provide RN Care Coordination through technical assistance, medical oversight, and training of staff to ensure extraordinary care
• Use your critical thinking and clinical judgment to best meet each individual's mental and physical needs
