PATH is seeking candidates passionate about helping others make a positive change in their lives to join our DHS ICMS Street Outreach team as the Lead Outreach Case Manager at the RHC Office.

ABOUT OUR TEAM
Our Street Outreach programs to work identify and build relationships with individuals currently experiencing homelessness and living on the streets. These programs utilize PATH's Street Outreach Integrated Care approach to assist clients in breaking the cycle of homelessness by providing field-based interventions, case management and supportive services. The DHS ICMS Program is comprised of a multidisciplinary team and conducts daily street outreach within the Los Angeles Metro transit system throughout the Metro Los Angeles region.

WHAT IS A LEAD OUTREACH CASE MANAGER
As part of the DHS ICMS Street Outreach, the Lead Outreach Case Manager utilizes their knowledge and skills to help ensure and provide high-quality services within the program that assist people experiencing homelessness obtain and maintain stable housing.

Position Responsibilities:

  • Conduct daily street outreach to develop relationships and build rapport with clients to assist them with finding a home.
  • Respond within specific contract timeframes to requests for street outreach services and send follow-up information to the requestor and supervisor.
  • Conduct VI-SPDAT (Vulnerability Index- Service Prioritization Decisions Assistance Tool) on all new clients to identify and connect them with proper intervention and services; document results, complete match initiation form and submit all paperwork to CES coordinator.
  • Collaborate with clients and the street outreach team to develop an Individualized Service Plan (ISP) for each client that addresses short- and long-term goals.
  • Provide street-based case management services that connect clients to long-term supportive services and help improve their quality of life:

WHAT YOU BRING
We're looking for candidates that:

  • Demonstrated knowledge and experience with advanced and evidence-based interventions including; Engagement and rapport building with hard-to-reach clients; Harm Reduction and Motivational Interviewing strategies; Housing First; Non-violent crisis intervention techniques; Case management interventions proven effective with vulnerable populations.
  • Demonstrated team leadership skills
  • Ability to work effectively independently and as part of a team.
  • Ability to exercise mature and professional judgement.
  • Strong problem-solving and conflict resolution skills.
  • Strong written and verbal communication skills.

PREFERRED QUALIFICATIONS

  • Three (3) years of experience in outreach, healthcare, case management, homeless services or working with vulnerable populations.
  • Lived experience with homelessness and/or issues related to homelessness and experience in street outreach.

MINIMUM QUALIFICATIONS

  • (Three) 3 years of related experience or educational equivalent

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

  • Ability to work the 3:00 pm – 11:30 pm shift five days a week
  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Must demonstrate a commitment to diversity, equity, and inclusion through inclusive service delivery, continued learning and development, modeling inclusive behaviors, contributing to an inclusive work environment, proactively mitigating bias in internal and external systems as well as interactions with internal and external stakeholders
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test

    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

  • Have reliable transportation
  • A valid driver's license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

COVID-19 VACCINATION
Effective 11/01/2021 ll require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.

FLEXIBLE WORK
We are a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
