Job Summary:
We are seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our home health team. As an LVN with Sunlit , you will provide high-quality and personalized care to patients in the comfort and privacy of their own homes.
Responsibilities:
- Conduct comprehensive assessments of patients' physical, emotional, and social needs and develop individualized care plans
- Administer medications, wound care, and other treatments as ordered by the physician and according to nursing practice standards
- Monitor patients' conditions, document changes in their health status, and communicate with the physician and interdisciplinary team as needed
- Provide patient education and support to promote self-care and disease management
Qualifications:
- Current California LVN license
- Minimum of 1-year experience in a healthcare setting, preferably in home health or a related field
- Excellent clinical, communication, and organizational skills
- Ability to work independently and as part of a team
