Job Profile Summary

This role focuses on providing pharmacy related services in a hospital or retail setting. In addition, this role focuses on performing the following Pharmacy Services duties: Delivers pharmaceuticals. Includes professionals who are trained and sometimes licensed to dispense medicine/controlled substances. A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs. The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. A senior level role that requires advanced knowledge of job area typically obtained through advanced education and work experience. Typically responsible for: managing projects / processes, working independently with limited supervision, coaching and reviewing the work of lower level professionals, resolving difficult and sometimes complex problems.

Job Overview

This position works closely with other health practitioners to meet the various needs of our patients. Improves the health of patients by providing pharmaceutical care to individual patients and by coordinating the procurement, storage, preparation, and distribution of drugs to all patients. Provides leadership by facilitating the development, implementation, and maintenance of hospital and departmental systems that promote desirable patient outcomes. This position is involved in educating students, interns, residents, nurses, and patients. Participates in interdepartmental committees. This position may also be required to assume the essential responsibilities and perform the duties of a department leader.

Job Description

Minimum Qualifications:

1. Bachelor's degree in Pharmacy.

2. Pharmacist License.

3. Three (3) years of related experience.

Preferred Qualifications:

1. Doctor of Pharmacy (PharmD).

2. Completion of Pharmacy Residency program.

3. Experience in drug utilization review and analysis.

4. Experience with Electronic Medical Records.

Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

1. Supervises personnel in a service area during a scheduled shift.

2. Ensures that all work assignments are completed in the service area by the end of a shift.

3. Serves as primary preceptor for PGY1 pharmacy residents, cooperative education students, PharmD Residents, and pharmacy interns.

4. Coordinates review of policies and procedures related to pharmacy operations.

5. Identifies the need for and creates new policies and procedures.

6. Initiates and facilitates completion of drug use evaluations.

7. Provides continuing education for hospital professional staff (e.g., pharmacists, physicians, nurses).

8. Participates in periodic reviews of therapeutic classes of drugs and presents findings to committees.

9. Prepares and presents to the appropriate committees' evidence-based reviews of drugs requested for formulary addition.

10. Demonstrates clinical leadership through participation on hospital multidisciplinary committees, presenting at state and national meetings, publishing in refereed journals, serving as a lecturer at a school of pharmacy or allied health, and or maintains an adjunct faculty appointment at a school of pharmacy.

11. Participates in the professional development of pharmacy operational specialists and clinical pharmacists.

12. Identifies changes in drug usage (volume and patterns) based on published literature and Tufts Medical Center practices.

13. Gathers and reports data for one or more than one of the following Department of Pharmacy Quality Programs that relate to: People; Clinical excellence; Operational excellence; Financial performance and growth; Safety and quality; and Research.

Physical Requirements:

1. Frequent standing & walking, and lifting of 5-10 lbs.

2. May required lifting and carrying light loads, including boxes, equipment, and stooping or kneeling.

3. Requires manual dexterity using fine hand manipulation to operate prescription medications and computer keyboard.

4. Requires ability to see medications, computer screen, and reports.

Skills & Abilities:

1. Ability to work independently with minimal supervision and as part of a team.

2. Willingness and ability to learn and utilize new technology and procedures that will continue to develop in their role and throughout the organization.

3. Superior communication skills, including developing and giving presentations, both 1-1 and group.

4. Knowledge of integrated health networks.

5. Knowledge of health plan drug formularies.

6. Project and resource management skills.

7. Creative thinking and problem-solving skills.

8. Ability to understand the importance of and respect for the confidentiality of all patient information in accordance with applicable standards and regulations.

9. Knowledge of clinical best practices and standards for optimizing medication use.

10. Proficiency in Microsoft Office applications, including Access, Excel, PowerPoint and Word.

11. Ability to work with all ages, backgrounds, ethnicities, and life experiences.

12. A robust understanding of chronic health condition management and population management.

13. Motivational Interviewing proficiency.

14. Ability to prioritize and resolve critical issues efficiently and effectively.

15. Detail oriented, with strong organizational skills and multi-tasking abilities.
