Lcsw Or Mft Case Manager/Clinician

We are seeking a Case Manager/Clinician to join our team! You will provide recovery-oriented rehabilitation services in group or individual settings.

Minimum Qualifications: LCSW or MFT or close to getting your license preferred.

Responsibilities:

• Establish open lines of communication for individuals who were formerly incacreated
• Offer assistance to individuals with substance abuse issues
• Provide prevention-oriented counseling and case management
• Implement and review treatment plans
• Collaborate with additional treatment providers and placement agencies
• Ensure all documentation is completed in a timely manner

Qualifications:

• Previous experience in criminal justice, mental health counseling or other related fields
• Familiarity with community health, and mental health resources
• Excellent written and verbal communications skills
• Ability to build rapport with clients
• Strong leadership qualities
