Community Clinic of Maui, Inc. dba Malama I Ke Ola Health Center is a Federally Qualified Health Center (FQHC) and a fully integrated behavioral health, dental, and primary care system. Our 100+ professionals and providers serve the people of Maui at three clinics-two locations in Wailuku and one in Lahaina.

We have behavioral health providers who partner with the primary care clinicians to assist in a vast number of activities including warm handoffs for acute behavioral health crisis management, substance abuse counseling, tobacco cessation counseling, chronic disease self management counseling, and long-term individual and family therapy. We have been serving the Maui community since 1993. Since 2015, we have served over 11,000 patients of which 97% are living at or below 200% of the Federal poverty level.

PRIMARY FUNCTION: The Licensed Clinical Social Worker (LCSW) delivers consultation and co-management of medical conditions for our School-Based program on Maui and in a team-based and trauma-informed setting. This clinician provides psychological interventions to treat the psychosocial and behavioral aspects of chronic and acute illnesses.

This clinician applies behavioral principles to address maladaptive lifestyle and health behaviors within a primary care setting in accordance with the applicable professional, ethical, and legal standards. Additionally, this clinician conducts intake assessments, individual, and group psychotherapy for mental health and substance use disorders, as well as for patients with dual diagnoses. Team-Based Care 90 Day Introductory Period Continuous Learning On-The-Job and Internal Training Sessions External Training -- Paid for Time Medical, Dental, Vision, and 403(b) Retirement Plan and other awesome benefits Vacation, Holiday and Sick Benefits Competitive compensation package, base $84,482 and a $10K Sign-On Bonus ESSENTIAL FUNCTIONS: Provides short-term counseling that is goal-oriented to meet a specific need and to explore available options. This may include work with independence issues, identity crisis, and body image; involving the family as necessary. Works with the student, family and other team members to gather and assess information regarding the client's needs, mental status, support systems, and available financial, community and governmental resources.

Works closely with SBHC team to develop a comprehensive treatment plan that meets the student's identified needs and revises the plan as needed to insure optimal treatment benefit. Obtains needed services for the student and negotiates and promotes their concerns within the SBHC team as well as the community. Keeps SBHC team informed of available community services, programs, and changes in community, state, and federal programs and policies that directly impact the provision of services to students.

Documents behavioral health assessment results, treatment plan, intervention(s), and outcomes in health record. Collects and maintains behavioral health data for reporting quality improvement and performance indicators. Utilizes evidence-based instruments and interventions for substance abuse and mental health screening, assessment, and treatment.

Develops and implements small groups (i.e., youth experiencing grief and loss, trauma, depression, support for youth transitioning back to a school environment from a more restrictive treatment settings) Establishes a mechanism for technical assistance and training in child and adolescent behavioral health issues. Provides mental health expertise during staff meetings and case consults. Interacts with crisis intervention and student assistance teams.

EDUCATION AND EXPERIENCE REQUIREMENTS: Master degree in Social Work form an accredited graduate school of social work or Master degree in psychology or counseling LCSW or appropriate accreditation. State licensure. One year experience in individual and group counseling with children and adolescents.

KNOWLEDGE, SKILL, AND ABILITY REQUIREMENTS: Excellent communication and public relation skills Demonstrated ability to facilitate management of patients in clinical setting. Flexible, able to function independently but also works well as a team player. Ability to provide individual, small group, and family counseling services.

Ability to organize work and meet service objectives efficiently. Knowledge of state and federal laws and regulations regarding the provision of ambulatory behavioral health services. Knowledge of proper behavioral health record keeping.

Good understanding of psychodynamics of children, adolescents, and treatment strategies for common mutual health problems of children and adolescents.
