We are currently hiring a Landlord Liaison / SOAR Case Manager, to work in our Schoharie, Delaware and Otsego County Offices. As the Landlord Liaison / SOAR Case Manageryou would be responsible for:
- Participate in the Coordinated Entry meetings to keep current in client information, housing needs, and client needs for SOAR
- Seek and identify available housing units and collaborate with landlords and Case Managers to support client housing placement and retention
- Collect all medical documents regarding individuals applying for SSI/D. Complete SOAR applications for individuals who have a physical and/or mental disability. Document the application. Conduct regular follow-up, barrier removal and problem solving until all applications/processes/appeals have come to final resolution
- Participate in Catholic Charities meetings and functions, including: Staff meetings, Christmas Board/Staff event and other staff training or appreciation days, Annual Dinner.
Landlord Liaison / SOAR Case Manager Minimum Salary: $28.57 / Per Hour
Landlord Liaison / SOAR Case Manager Maximum Salary: $28.57 / Per Hour
Landlord Liaison / SOAR Case Manager Schedule: Monday – Friday Day shift
Qualifications:
- Associates Degree in Human Services, Law Enforcement or related field.
- Valid Driver’s license for travel within counties assigned.
