SUMMARY
The iPRTF Coordinator provides intensive case management services to the children and families identified for the agency iPRTF program. The iPRTF Coordinator assesses the readiness of the families for services, educates families on the recommended plan of care and partners with them to implement and monitor the best care plan for the child. The iPRTF Coordinator collaborates closely and frequently with the iPRTF Licensed Therapist to partner with the family in building and maintaining a support system for the client, working on the development of an integrated plan and establishing a strong relationship with the family. The iPRTF Coordinator will develop trust and mutual respect with the youth/family in order to create a shared vision and establish buy-in and shared vision.

Complete targeted direct service hours
The iPRTF Coordinator will have at least weekly contact with the families of current PRTF/RTC consumers. They will have at least weekly contact with the families of discharged clients during the first month after discharge. Biweekly contact the second month they discharge. They will have at least monthly contact with the families there after until a year after discharge.
Coordinates and facilities Child and Family Team meetings
Will plan and coordinate CFT meetings for all clients directly assigned to the iPRTF program in the parameters assigned by the treatment plan. If they are the case manager as well.
If they do not have case management responsibility, they will attend all CFT meetings. Whether they have case manager responsibilities or not, the family partner will communicate with the family prior to the meeting to gain an understanding of any concerns, needs or specific topics to be discussed at the CFT. Will advocate for family and child.
Ensures the Wraparound processes is completed with high fidelity to the model and tracks progress toward goals.
Will review progress towards goals monthly (?) and evaluate any changes in level of care.
Collaborate and advocate with service providers, state agencies, and community partners on the behalf of the youth and family to ensure that needs requirements met in a timely manner.
Will maintain a network of resources for clients and families and advocate for families as their needs change
Prepares summary documents and final reports
Reporting will be submitted within determined timelines 100% of the time.

90% of the time, will meet discharge plan outcomes
Completes documentation of services and treatment plans consistent with clinical and administrative policies and procedures. All documentation including progress notes are completed accurately and in a timely manner
100% of documentation will be submitted the same day as the treatment occurrence. 100% of progress notes are complete and accurate.
Maintain active service plan on each client
100% of events provided and billed are on active service plan
Engage client effectively managing no-show rate
Percentage of all client appointments that no-show after initial assessment is less than 10%
Accurately maintains client Electronic Health Record
Data entered in EHR is accurate 99% of the time
Complete all mandated training, including ongoing support and trainings from the NC Families United and Workforce Development
100% of required training will be completed within specified time frame
Advocates for the family. Including thinking of strategies and ideas to help the family and the child feel safe and connected during treatment. Assist the family with understanding the coping strategies learned, structure and preparation for discharge. Will provide One on One Family Support, Provide advocacy, Navigation of Service Delivery System, Provide Family Driven Voice, Promote Family and Agency Partnerships. Will begin developing the relationship and connection to that family. They will discuss openly their child, their goals for treatment, and gain an understanding of Alexander Youth Network and their treatment practices. Will provide input to the admission team. There will be ongoing support offered to the family and conversations with the family and child regarding routine, what's working and what is not; Asking questions like what helps you go to sleep, what are night time routines, do you have a favorite food, when stressed, do you like people around you?
100% of documentation will be submitted the same day as the treatment occurrence. 100% of progress notes are complete and accurate.
Engaging in relationship building. At a minimum, the family partner will have weekly contact with the family via in person, telephone or video chatting, while the child is enrolled in services
Will develop an understanding their home environment and structure to begin individualized plans for successful discharge. They will develop a Futures Plan for the family that assist with identifying needs, natural supports and dreams for their family. 100% of documentation will be submitted the same day as the treatment occurrence. 100% of progress notes are complete and accurate.
Attend all required meetings related to Six Core Strategies, BBI, and Family Partners.
100% of meetings will be attended, unless 24 notice is given
Post discharge will maintain contact
At least one contact per week for the first week. At least one contact per two weeks for the second month. At least one contact per month after that.
QualificationsQUALIFICATIONS

EDUCATION and/or EXPERIENCE
A Bachelors' degree in a human serves related field; Master's degree preferred. At least two year of experience providing services for children/families. Must meet the NC state minimum qualifications for a Qualified Mental Health Professional, if they will have case management responsibility
If not already certified, must complete national certification as a family peer partner within 6 months of hire or have a plan approved by your supervisor that includes active trainings attended to complete the certification
Experience working with or living with children who have complex emotional needs/ knowledge of community resources preferably experience with local resources.
TRAVEL
Frequent travel required within NC. Must be able to travel between Alliance catchment area and Charlotte-based PRTF facility.
LANGUAGE SKILLS
Ability to read, analyze, and interpret professional journals, governmental regulations, or legal documentation. Ability to write reports and business correspondence to a prescribed format and style. Ability to effectively present information and respond to questions from groups of children, senior managers, clients and families, volunteers, and the general public.
MATHEMATICAL SKILLS
Ability to work with mathematical concepts such as probability and statistical inference. Ability to apply concepts such as fractions, percentages, ratios, and proportions to practical situations.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
CERTIFICATES, LICENSES, REGISTRATIONS
Valid NC or SC Driver's license. Must maintain minimum state requirements for auto insurance and does not fit the criteria identified as a High-Risk Driver.
