Job Description
Intensive Care Coordinator - Davis/Morgan Counties
(Caseworker II)
This position fills the Intensive Care Coordinator role in Davis and Morgan counties region Utah Department of Human Services works to ensure children, families and adults in Utah communities are receiving the support they need to lead safe and healthy lives. HFW Facilitators (Caseworker II) exercise independent judgment and discretion in assessing and providing direct in-home and community based support through the HFW process; recommend appropriate intervention, document all activities on behalf of families and youth, complete assessments and prepare other related reports; develop wraparound plans and crisis prevention plans and document progress; may coordinate provider services and ensure the quality of services for youth and families; may perform on-call services under supervisor's direction, as needed.
  • May require Utah licensure as a Social Service Worker.
  • Some positions may require previous experience with the community or with children who have complex needs.
  • This position may work in others areas depending on workload.

Principal Duties
  • Learn and follow the High Fidelity Wraparound model as a facilitator (initial and ongoing training provided); this includes conducting meetings with families and required documentation
  • Will facilitate high fidelity wraparound services for families: utilize a strengths based approach to safety planning, development of family team and family support systems, and wraparound planning for the purpose of maintaining children in their homes
  • Provide regular communication and close collaboration with multiple community partners (e.g. Juvenile Court, Division of Juvenile Justice Services, Division of Services for People with Disabilities and Local Mental Health and Substance Use Disorder Treatment Facilities)
  • Coordinate and refer services of community resources, placement providers, collateral agencies, the court, and/or other community partners with families or youth receiving services
  • Prepare detailed case documentation including: opening and closing referral records and completing assessments, Strengths Needs & Cultural Discoveries, Wraparound Plans, contact notes, safety and crisis prevention plans, and correspondence, etc.
  • Prepare wraparound plan with families and agency partners with measurable outcomes and time bound specific objectives
  • Provide families with information concerning the wraparound teaming process, resources, supports, services, goals and expectations
  • Provide the facilitation of high fidelity wraparound and follow through for youth involved in multiple systems
The Ideal Candidate
The model candidate for this position is someone who:
  • Has 2 or more years experience of human services experience including juvenile delinquency and/or child welfare; writing court reports, and/or working with community partners involved in the juvenile justice or child welfare process.
  • Has Bachelor's level degree OR the equivalent to a Bachelor's in experience in any human services related field with a minimum of two years of full-time work providing direct services to families of children, OR an Associate's degree or the equivalent to an Associate's in experience in any field with a minimum two years of providing High Fidelity Wraparound to families.
  • Has experience implementing High-Fidelity Wraparound.
  • Has an active Social Service Worker license in the state of Utah.
  • Is passionate about helping families receive the support and services they need.
  • Works with people in a manner which shows respect, sensitivity, tact, and professionalism
  • Loves interacting with community agencies.
