Intensive Care CoordinationFamily Peer Support Specialist
(Northern Region)

Would you like to provide support to families in the Northern Region of Utah in caring for children, youth, and young adults with mental health and/or substance use challenges? Do you want to help youth and families in elevating their voice and choice, while also helping them to understand the perspective of others, gain skills around independence and self-efficacy, all while working with a multi-system team? If yes, then this role as the Intensive CareCoordination Family Peer Support Specialist is the role for you!
Job Responsibilities:

  • Partner with the Intensive Care Coordinator using the High Fidelity Wraparound Model
  • Engage and orient the family in using the Intensive Care Coordination High Fidelity Wraparound Model
  • Support the family and natural supports using the High Fidelity Wraparound Model and any Crisis Plans that are created in the process
  • Support the youth and families throughout the process in building skills that will support a smooth transition from Intensive Care Coordination
  • Complete daily documentation on any face-to-face contacts, meetings and coordination of care with the family and team members
  • Work with the youth and family in their homes and in the community
  • Support and encourage prosocial, skillful behavior amongst the youth and families while maintaining healthy boundaries
  • Implement "Do For," "Do With," "Cheer On" with youth and families while supporting them in focusing on the Theory of Change

Ideal Candidate:
  • Strong understanding of the community, resources in the community, and must be able to provide support in an ethical and culturally competent way
  • Experience as a parent, family members, or caregiver to a youth with mental/behavioral health or substance use disorders and who were involved in systems
  • Willing to attend any onboarding trainings, as well as an Intensive Care Coordination training
  • Able to input data into a medical health record, use appropriate grammar and sentence structure, and store family files on the approved DHHS platforms
  • A valid Driver's License will be required to transport families, as well as completion of the State required defensive driving courses

Preferences:
The following experience or skills may be of benefit to you, but the absence of them will not, in any way, exclude you from consideration. The hiring manager may identify additional related skills and requirements as preferences in making hiring and promotion decisions.
  • Experience with systems and/or teams
  • Proficiency in using the Google platform and in writing notes/documentation
  • Experience using the High Fidelity Wraparound Model

  • This position may currently be a hybrid of both in-office and remote work days. Please note, a position's eligibility for remote work is established by agency management and is subject to change at their discretion at any time and for any reason.
  • EffectiveJuly 1, 2021, State of Utah Employees must either reside in Utah or be in the process of relocating to within 30 days of their start date.
  • Risks found in the typical office setting, which is adequately lighted, heated and ventilated, e.g., safe use of office equipment, avoiding trips and falls, observing fire regulations.
  • A valid driver's license may be required if employee is required to transport clients.
  • Depending on the area of assignment, some positions may require previous experience with the community or client base served by the position.
  • Typically, the employee may sit comfortably to perform the work; however, there may be some walking; standing; bending; carrying light items; driving an automobile, etc. Special physical demands are not required to perform the work.
