Please note that individual total compensation for this position will be determined at the Company's sole discretion and the wage range for this role considers a wide range of factors including but not limited to skill sets; experience and training; licensure and certifications; and other business and organizational needs. At SNAHC, it is not typical for an individual to be hired at or near the top of the range for their role and compensation decisions are dependent on the facts and circumstances of each case. A reasonable estimate of the current range is $66,031-$89,336.
Position Summary:
The Integrated Behavioral Health (IBH) Provider-ACSW reports to the Behavioral Health Program Manager for administrative task and to the Clinical Director for all things clinical and is responsible for attending to clinical requests for behavioral interventions for a variety of patient issues including: depression, anxiety, pain management and other chronic conditions for which the patient would benefit from behavior changes, coaching, and referrals to supportive programs. The IBH Provider may also intervene for crisis response, or to support the patient and care team in establishing behavioral norms for the continued care relationship. The IBH Provider provides brief, solution-focused interventions, follow-up, and referral to improve patient health outcomes. The IBH Provider contributes equally to the patient care team through case consultation, patient advocacy, and collaborative team meetings with Clinical staff.
Essential Functions:

  • Provides brief, problem, and solution-focused interventions to address primary mental health concerns and/or supports behavioral changes to improve outcomes of treatment plans.
  • Recognizes and treats mental health and psychosocial issues as they arise. Serves as a critical contributing member of integrated care teams with medical providers, medical assistants, other clinical support staff, and patients.
  • Provide individual therapy and maintain a panel of patients.
  • Consults with and provides training to care team members to enhance communication skills, cultural sensitivity, and effectiveness in managing behavioral components of health and mental health conditions.
  • Acts as lead in behavioral health consultant and manages all requests for consultation and triages needs according to mental illness severity, including severe.
  • Provides patients with self-management skills and tools to enhance their participation in their own treatment and recovery. Collects, develops, and distributes health and mental health improvement tools on an on-going basis.
  • Using best practices teaches patients skills to manage chronic conditions.
  • Provides patients with support, instruction, and modeling for behavior befitting the clinic setting and most likely to encourage mutually respectful care.
  • Identifies high-risk, high-needs patients for greater attention to whole-person care and health improvement planning.
  • Performs risk assessment and crisis management, including evaluation and referral to Emergency Services for patients who are suicidal, homicidal, or experiencing intimate partner violence.
  • Gives timely feedback to clinical providers regarding care plans, treatment recommendations, and progress, verbally at the time of visit. Attends and supports regular collaborative team meetings to discuss both patient issues and team collaboration issues.
  • Documents all patient interactions in the Electronic Health Record according to agency procedures and timelines.
  • Makes referrals to specialty mental health and other specialty services within the agency as appropriate including but not limited to Pathways Whole Person Care Program.
  • Assists patients with tasks associated with community functioning, such as filling out paperwork for public benefits, referral to higher level of care and supportive programming, vocational rehab, social support, housing, food and clothing closets, etc.
  • Provides ongoing education and consultation to greater Health Center and community on the role and benefits of Integrated Behavioral Health. Makes recommendations on outcomes and program evaluation.
  • Assists with data collection and other duties as assigned by Behavioral Health Leadership team.
  • Complies with all state and federal laws and regulations, as they pertain to position including; HIPAA, sexual harassment, Scope of Practice, OSHA etc.
  • Other duties as assigned.

Skills and Abilities:
  • Excellent computer skills, preferably with Windows, including Microsoft Office Suite
  • Excellent telephone and communication skills
  • Must possess excellent organizational, writing, and verbal skills.

Minimum Qualifications:
  • Registration with the Board of Behavioral Sciences of California-ACSW
  • Master's degree in social work
  • Experience working in mental health field
  • Knowledge of common behavior change theories and issues facing the clinic population, able to utilize evidence-based interventions to improve client outcomes.
  • Training in de-escalation techniques
  • Experience with brief interventions, such as Motivational Interviewing, Harm Reduction.

Preferred Qualifications:
  • Experience with Nextgen or other electronic health record system.
  • Knowledge of Third Party and/or FQHC billing.
  • Experience in an FQHC setting
  • Knowledge of traditional, cultural, and spiritual practices of the diverse AI/AN community, as well as ability to work with other racially, culturally, ethnically, and financially diverse populations.
  • Experience using evidence-based treatment modalities.
