Intake Referral Coordinator

JOB RESPONSIBILITIES:

1. Process incoming referrals
a. Process incoming referrals and verify that the information received is accurate and complete
b. Verify correct information for referral based on Intake Matrix
c. Route referral in Epic to correct department work queue
2. Identify initial medical records needed
a. Use scheduling Matrix to identify medical records that are needed for medical review or patient visit
b. Send initial request(s) for those records
3. Communication
a. Communicate with referring practice if information is incomplete, inaccurate or additional information is needed
b. Communicate with supervisor/manager if information
4. Monitoring and quality assessment
a. Identify barriers to referrals being processed appropriately
b. Identify system or process improvements
c. Keep management apprised of issues, challenges and barriers
5. Diagnosis matrix
a. Regularly review and recommend updates to the Intake Matrix
b. Ensure accuracy of details on the Matrix. Work with Supervisor if updates are needed

Job Type: Contract

Pay: $21.00 per hour
