REPORTS TO: Program Manager/Director
CLASSIFICATION: Non-Exempt
Job Type: Full-time

SUMMARY: Step Up on Second, a nationally recognized non-profit mental health and housing provider currently has an opportunity for a Intake Coordinator in Savannah, Georgia.


The Intake Coordinatoris a Georgia licensed LCSW, LPC or LMFT whose primary role includes completing bio-psychosocial intakes and ANSA assessments, treatment planning and therapeutic service provision to homeless adults with psychiatric disabilities in community settings. The Intake Coordinator is part of a team providing field-based Medicaid billable Housing Supports, Case Management and clinical services with goal of moving adults experiencing homelessness and struggling with psychiatric and co-occurring disabilities into permanent supported housing and towards independence, community integration and long-term housing retention. The team consists of Housing Specialists, Case Managers, Peer Specialists and Clinicians utilizing the Housing First, Trauma Informed and Harm Reduction models of service delivery to clients with the Georgia Housing Voucher (GHV) in community settings.
RESPONSIBILITIES: The following reflects essential functions for this job but does not restrict other tasks, which may be assigned.
Field-based Direct Service and Client Care:
· Complete comprehensive Psycho-social Evaluations, ANSA, Initial Diagnostic Assessments, treatment plans and provide mental health services to homeless members in the community.
· Provide client-centered support using Evidence Based Therapies such as Harm Reduction, Motivational Interviewing, and Trauma Informed Interventions.
· Complete all chart paperwork and maintain documentation in Electronic Health Record – Welligent within prescribed timeframes as outlined in SUOS documentation policy.
· Works with an interdisciplinary team including a Case Managers, Peer Specialists and community providers.
· Other Duties as assigned.
Quality Assurance and Reporting:
In coordination with the Step Up management and Quality Assurance staff, implement and follow Quality Assurance activities to meet Medicaid Standards and protocols; monitor Electronic and paper documentation and charts; determine and review appropriate use of program services and accuracy of documentation with team.
SKILLS: Knowledge of or experience with DSM diagnoses, assessment of level of functioning, EBP’s such as Motivational interviewing, DBT, Cognitive Behavioral Therapies, Trauma Informed Care, Housing First, etc. Ability to work independently and collaboratively on a team. Uses good time management skills and resources to balance case load direct service and paperwork. Knowledge of Office 365 applications and Electronic Health Record systems.
QUALIFICATIONS: Master’s Degree; LCSW, LPC or LMFTin good standing with the Georgia Licensing Board required.Experience working with homeless adults experiencing mentally health issues in a community setting required. Experience conducting intake assessments in Medicaid billable projects preferred. Candidate must have own transportation; current Georgia driver’s license and insurance with an excellent driving record.
SUPPLEMENTAL INFORMATION: Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty. Additionally, in working with our members, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise and Step Up has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.
PHYSICAL REQUIREMENTS: While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.
