Title: Intake Assessment Clinicians

Department: Assessment & Referral

Location: 3955 156th Street NE, Marysville, WA 98271

Position Description:

The Intake Assessment Clinicians is responsible for coordinating referrals and other requests for services, information, and admission under the direction of the Director of A&R. 

PRIMARY RESPONSIBILITIES

1. Inquiry calls are documented and processed in accordance with Intake policies.
2. Patients are assess and stabilized prior to leaving hospital or admission to hospital arranged 
3. On call doctor is notified and assisted with patient interview.
4. Admission paperwork is completed in accordance with hospital policy.
5. Intake log is completed with all information documented.
6. Follow up calls are made to those failing to show up for appointments.
7. Pre-certification is obtained in accordance with insurance requirements.
8. Outcome studies forms are completed.
9. Guest relations with referral partners & walk-ins 
10. Completion and accuracy of EMTALA log. Processing patient belongings & valuables. Processing internal labs, verifying insurance and chart building.

Education:    Master’s Degree in a human relations field is required.

Experience:  Preferably experience in Intake Assessment activities within an acute psychiatric setting. 

Licensure: Licensed to practice in the State of Washington as a mental health professional (Social worker, Counselor or RN with MHP).

Additional Requirements: A working knowledge of the latest DSM and Mental Status Exam. A working knowledge of the Addiction and Recovery field.
