Position Available:

• In Home Supports Specialist (Part Time and/or Full Time Day Position with Hiring Bonus)

Service Location:

• Caseload #1 - Dakota and Ramsey County

Job Duties: Assists the consumer in developing, maintaining and improving his/her daily activities by providing weekly individualized support to live successfully in the community. Assists the consumer with accessing and navigating through community resources. Accompanies the consumer on outings within the community. Provides weekly updates to the consumer's interdisciplinary team members regarding the consumer's progress.

Qualifications

• High School Diploma
• Valid Driver’s License and Up to date Auto Insurance
• Maintain acceptable motor vehicle report
