Constructive Partnerships Unlimited (CP Unlimited) is a comprehensive multi-service and multi-site organization providing services and programs to people with intellectual and developmental disabilities throughout the NYC metropolitan areas.
CP Unlimited operates an extensive community based living program, day habilitation programs, community habilitation services and provides rehabilitative services in a caring and comfortable atmosphere. CP Unlimited also provides nursing, behavioral, dietary and Article 16 services including counseling, physical therapy, occupational therapy, and speech therapy. Well-coordinated care, overall wellness, and building personalized relationships with the people that reside and participate in our programs are our high priorities.
Currently CP Unlimited is seeking a full time Incident Management Investigator in our Quality Assurance Quality Improvement Department to advocate for the the rights of all people receiving services throughout the Agency by conducting investigations into situations that may violate a person's rights and/ or protections in accordance with OPWDD and Justice Center regulations.
Specific duties include:
- Conducting investigations into situations that may violate rights and/or protection of people (e.g., allegations of abuse, mistreatment, neglect, and violation of civil or human rights)
- Ensures investigations are timely, thorough, and address investigatory issues, as well as identify best practices in accordance with agency policies and regulatory standards.
- Identifies areas of concern timely during the course of an investigation needing additional follow-up and/or additional reporting.
- Prepares investigative reports for organizational personnel and outside agencies (OPWDD IMU, CAB,MHLS, Police Justice Center, etc.)
- Presenting summary of investigative findings to the Incident Review Committee
- Conducting training sessions in abuse identification, prevention and reporting
Requirements:
- Bachelor’s degree in Human Services
- Minimum of 3 years’ experience in conducting investigations involving OPWDD programs
- Experience working within OPWDD programs required
- Must be able to travel throughout the five boroughs of NYC
- Must have flexible hours and ability to work remotely and on site as needed.
